TRANSITION SERVICES
Transition is an individual journey, which is why at CDS Monarch, the focus is on the individual; his or her specific abilities, and personal goals and dreams.
CDS Monarch recognizes a gap in services for individuals with developmental disabilities that exists between the time they are able to graduate and the time they become eligible for adult services. Because of this, we have chosen to take the lead, in the delivery of transitional services.

CDS Monarch's Transition Program provides supports to young adults, ages 16-21, who have recently graduated or “aged-out” of the school system or who are going through a life transition into a new program and/or employment or housing environment.
Partnerships have been developed with local schools to provide person-centered transition supports to those students who have not yet aged out school. Key focus areas may include employment skill building, decreasing levels of supervision and building of social skills. Transition services are expected to last for a period of approximately one year and with the duration being tailored to the needs of the individual.
