Medicaid Service Coordination:
CDS Monarch provides Medicaid Service Coordinators who work hands-on with individuals to assist them in achieving their goals and dreams. We have over 20 years of combined service coordination and case management experience. Service Coordinators have experience working with children, adults and seniors, including individuals on the Autism Spectrum and high-needs individuals. They are also knowledgable about residential programs and

requirements, so whether an individual lives alone or in an IRA, CDS Service Coordinators will be able to assist.
An Individualized Service Plan (ISP) is developed for each individual and their circle of support, outlining who they are, what they hope to accomplish and what strategies and supports are in place to help them achieve those dreams.
Our Medicaid Service Coordinators assist over 650 people, living in Monroe, Wayne, Ontario and Livingston counties, with accessing a variety of services including: future planning, recreation, Individualized Day and Residential planning, Family Reimbursement and Environmental Modifications. Plans are monitored and evaluated to help support individuals as they become their own self-advocate. CDS Monarch maintains a robust self-auditing process which exceeds standard regulatory requirements.

Traumatic Brain Injury (TBI) Service Coordination:The focus of CDS Monarch's Traumatic Brain Injury (TBI) Service Coordination program is to assist those individuals who have sustained a traumatic brain injury with accessing the necessary supports and services to live successfully in the community. TBI Service Coordinators assist with accessing funding for housing and utilities, along with bringing various supports & clinical services into their home. TBI Service Coordinators may also assist people in obtaining community based services to help with recovery.
Self Determination or Consolidated Support Services (CSS):Self Determination or Consolidated Support Services (CSS) is an alternative to how traditional services are delivered. If you or your loved one have tried different programs and services in the past that have not worked but the services are still needed, then you might be interested in what CSS has to offer.
CSS is a self directed service that represents a partnership between an
individual and the Agency that provides support and services to the individual. The program is self directed - the individual has authority over his/her budget and the authority to hire his/her own employees. CSS is a Medicaid funded service that is based on the individuals valued outcomes.A number of Medicaid Service Coordinators at CDS Monarch have obtained their broker certification for the CSS program and are ready to assist individuals with this service.
