Transition Services

Our new and exciting Life Transitions Center is established upon the foundation of more than thirty years of outstanding service to people who have a developmental disability.  This program is a unique and innovative response to the needs of families who have been frustrated by the lack of transition services in our community.  Our clinical approach differs from traditional services as staff focus on helping young adults to transition from school to work and appropriate community living.

The vision of the program is:
- To meet the needs of the 18-22 year old developmentally disabled high school students who are aging out of the public school system
- To support the life transitions of all OMRDD eligible individuals with developmental disabilities

The program works with each individual and their family, school district or agency to support the pursuit of each person's personal dreams.

Highlights of the Center include:
- An inviting Town Square
- A complete model apartment equipped for assessment & training
- Aquatics Therapy Center
- A modern restaurant operated by individuals with developmental disabilities
- Satellite Banking
- Fitness Center
- Media Center
- Gardening skills development
- Blended Day Habilitation/Vocational Services
- Creative Arts skill development (dance, art, music, drama, ceramics)
- Wellness Center

The CDS Life Transitions Center will bring together the above community oriented activity centers under one roof for the purpose of helping high school students and young adults transition to community living with opportunities for paid work experiences.
